Welcome to The Cancer Support Center
All of our programs and services are offered completely free of charge — insurance is never billed. As a licensed mental healthcare provider, we do ask that all new participants complete a brief set of consent and information forms before joining programs. These can be completed in advance using the PDFs below, or in person at either of our locations.

For Adult Participants
Adults joining any Cancer Support Center program are asked to complete the following forms. You may download and print them, or complete them when you arrive.
For Child & Teen Participants
Children and teenagers participating in CSC programs have their own combined consent and information form. A parent or guardian signature is required.
COVID-19 Consent Forms
For in-person visits, participants are asked to complete a COVID-19 liability waiver. You may complete the printable PDF or fill out the online version.
